In Luzerne County they only check your gas cap for emissions insp. and they only do a visiual for the polution control under the hood. Fat people are hard to kidnap!!! Depends on the county and I don't know what York county requires, I believe a neighboring county in PA has no emissions testing whatsoever (it's based on population density I assume).
