New Clutch Pivot Broke After Just 750 Miles!

Just like the title says, a brand new OEM pivot ball from the dealer broke off at the stud just 750 miles after being installed. Clearly I must have done something wrong during installation, any ideas? I don't want to go back in to the bellhousing and install a new one just to have it break again.

OK pulled it all apart in record time last night. Clearly the pivot had backed itself out of the bolt hole and then broke when torqued- the remaining stud in the bolt hole basically came out with the lightest of touches on the extractor (phew!).

 

I didn't buy a new pivot, I used the old one since it looked fine and operated with no issues. I had a hell of a time finding a torque spec for that, I came across an install video for an aftermarket pivot and they said 12ft lbs.....no effing way I was going that light again. I used some red threadlocker and put it on to more like 25 ft lbs. Also this time I had a 19mm deep well socket that my friend gave me which helped enormously.
